  • Always call one week before you run out of medication.

  • Call the Pediatric or Family Practice Department (phone numbers listed below) where your child is seen and please have the following information ready:
    • Name, age, weight and chart number of the child

    • Name of CMG Pediatrician or Family Physician


    • Amount and times medication is taken

  • Please allow two to four days for the prescription to be prepared.

  • Pick up the prescription at the nursing station of your child’s physician.
